I normally never use anything made of cotton, but when it is really cold...I sometimes will use a pair of sweatpants over my poly long john bottoms and wear my Breathable waist-high waders over that. Has kept me very warm while wading even in some pretty cold water and even my feet stay pretty toasty.
For the top I just use layers begining with the quick dry long sleeve tops that keeps the body moisture wicked away from the body. How much layering and with what depends on just how cold it is.
Follow Cook's advice on headwear...it can be the difference on a cold bitter windy day. Haven't really found the right kind of gloves yet :?

Breathable waist high waders with neopreme booties. Long sleeve shirt with rain jacket. If really cold, I break out the polarfleece.

Anything below 70 is freaking freezing as far as I am concerned. I have a sit in side POS that no self respecting kayak fisherman would be caught dead in but, I have had it for about 8 years now and when I got it I didn't know any better but, I will say this I am always dry and warm. I am usually looking for a way to cool off.

I am not a rum drinker. Give me a good scotch or bourbon.
